The Quality and Safety education for nurses initiative was developed to integrate quality and safety competencies into nursing education.The current challenge is for nurses to move beyond the application of QSEN competencies to individual patients and families and incorporate systems thinking in quality and safety education and healthcare delivery.The overall goal for the quality and safety education for nurses is to meet challenges of preparing future nurses who will have the knowledge,skills and attitudes necessary to continously improve the quality and safety of the healthcare systems within which they work.By the time nursing students graduate,they should have acheived theses competencies and should continue to address and utilize them in their professional practice role.After nursing students graduate,they look to experienced RN as their preceptors and mentors,and to nurse managers for guidance throughout their transition,which is why its crucial that theses competencies are comprehensed and embraced by all nurses.

Care management is a set of activities intended to improve patient care and reduce the need for medical services by enhancing coordination of care,eliminate duplication and helping patients and caregivers more effectively manage health conditions.These efforts have demonstrated potential to improve quality and control costs for patients with complex conditions.Care management within integrated speciality groups improves quality,but does not consistently reduce costs.The most effective care management programs are those targeting patients discharged from hospitals.

• Traning and personnel:programs with specialy trained care managers who have relatively low workload are most successful.Most care managers are registered nurses who work as part of a multidisciplinary team.
• Physician involvement:Placing care managers with physicians in primary care practices may help facilitate physician involvement.
• Informal caregivers: Patients with complex healthcare needs,particularly those with physical or cognitive functional decline,often need the assistance of informal caregivers to actively participate in care management.
• Person to person encounters including home visits are necessary features of effective care management.care management relying solely on telephone encounters has not shown success.
