SITUATION :

My name is Priya Rai and I am from the emergency unit. This call is regarding the patient named Jonah, room number 201. Patient came with the complaints of breathing difficulty.

BACKGROUND :

Patient shows signs of respiratory distress as lips are cynotic and use of accessory muscles are seen. There is no medical history known.

ASSESSMENT :

On assessment the blood pressure was 100/70 mmHg, temperature was 98.7 degree, Pulse was 130 beats/minute, and respiration was 28 breaths/ minute. The patient was not on oxygen. Ony lips were cynotic.

RECOMMENDATIONS:

  • The patient should be transferred to the pulmonary intensive care unit (PICU).
  • The doctor should be paged regarding this as it is an emergency case.
  • The relatives should be restricted to meet the patient.
  • Lab tests such as hemotology, ABG analysis, bronchoscopy, X-ray chest AP shoud be ordered ASAP.
  • Patient should be put oxygen.
  • Semi fowlers position is recommended.
  • Administer medication as per physicians order.
