The major aim of the patients consent form to create an opportunity to informed participant in his healthcare decisions. The type of decision for the relevant risks, benefits and uncertainties related to each alternative.

The importance of factsheet for patient or his or her family member:

• To know details of your body and it working and what is happening to body right now.
• To know about the different medical terminology related to different diseases so that he can communicate with any other persons who are linked with the healthcare fields.
• To understand the detail of the available options for the treatments and where choices are limited - especially what is in my control and what is not in my control
• To know details about the tradeoffs and choices of action that patients or doctor can take to address the issue or problem well enough to make a decision about what I or my doctor should do.

The patient’s factsheets should contain only essential information which can useful to the patient or family member make a specific decision or take a specific action. Too much amount of information can be inundated to the patient or family member of patient which can stop the person from reading and understanding. So, all the information should be presented in a single, focused which involves an immediate action or decision. The details of the future outcomes and required actions should be kept separately. The included information should be action oriented, whenever possible, presented in terms of actions taken by a clinician, patient or family member to address the medical need.

Factsheet should be in very simple languages and can be understandable to common peoples also.
