a. You will need an informed consent process as part of your program evaluation. How will you obtain informed consent from your participants? Will you have them sign a form or just verbally agree to participate in the evaluation?

Informed Consent : Informed consent is a process of taking ones permission to perform a procedure or evaluate a person.Here the person gets an ultimate freedom to decide himself whether to go through a particular procedure or not.If the consent has not been obtained the opposite person either health care provider or a researcher could not evaluate or perform anything.

So ,I would like to get a consent form signed rather than getting permission verbally.Because a verbal consent will not have any documented proof which may further lead to legal issues.

And I will try to get signed from the person by explaining the details of the programme.

Will clearly mention the benefits of the evaluation either rewards or recognition etc.

Also I will educate about the intellectual usages of the programme.

Assures the person regarding no harm and no threat from the form signed and explains as it is only the permission we are taking from the person.

These are some of the steps I will follow to get consent from my participants.
