11. The ICD-10 CM and ICD-10 PCS coding manual is maintained and updated by the NCHS (National Center for Health Statistics) of the CDC (Centers for Disease Control and Prevention) and CMS (Centers for Medicare and Medicaid Services) to use in the US. ICD-10 CM is based on ICD-10 formulated by the WHO (World Health Organization) and is used internationally. A representative of the NCHS and a representative from the CMS share this responsibility of the ICD-10 updating and maintaining this through Coordination and Maintenance Committee meetings. NCHS leads the responsibility for ICD-10 CM and CMS leads the responsibility for ICD-10 PCS.

12. The standard billing claim form for all outpatient services is CMS-1450 (UB-04).

13. Healthcare clearinghouses are electronic hubs that allowed the transmission of electronic claims of healthcare practices to insurance carriers in a safe and secure manner that protected the patient healthcare information. These hubs checked the claims for errors and verified the payer software compatibility, which is referred to as claims scrubbing.
