CLIENT HAS DIFFICULTY OPENING MEDICATION BOTTLES

ASSESS AND OBSERVE its the initial step , here we assess the situation that our patient having the problem related to the skipping theday to take medication , as he is suffering from the glaucoma whose mainly clinical manifestation of blurred vision and confusion in mind which gave impact on forgeting to take medicine on saturday and sunday .

CONTACT In this s tep we notify this all problem of our patient to our supervisior and inform that the client is having the difficulty in opening the medication bottles .

TALK Here we have to talk to teh patient family member ( if present) , and to advise to give on attention on the patient's medication taking schedule .,

IDENTIFY here we help them by follow agency policy on medication procedures . that helps in taking it at the right time , with right method .

OPEN here we open thechart and documented that what we have done on solving the client problems.

NOTE we have to note that in the next follow up visit to see if the client is still having problem related to opening the medication problem or not .
