#3 You are providing home care for a 55-year-old patient with hypertension, diabetes, and a new diagnosis of vertigo.
A) Identify the priorities, approach, and techniques you would use to provide nursing care to this patient.
B) How would your priorities, approach, and techniques change if the patient was 85 years old and lived alone?
A) Generally diabetis should be given first prioty, hypertention second and vertigo third
For DM The blood sugar has to be checked with glucometer when ever needed. regular food and medications are to be given. make sure patient takes food after medicine is taken specially if he is taking insulin. generally watch for hypoglycemia.
For hypertention, the BP has to be monitored daily. Avoid high calorie diet and encourage salt restricted diet. make sure patient take all medicine and night dose should be taken atleast 2 hrs before sleep. watch for hypotention and CAD.
For vertigo advise patient to be in bed rest and proper medication
B) In case of old age the newly diagnosed vertigo has to be given more priority. advise complete bed rest and avoid head turning. always mobilise patient in wheel chair. Dont allow patient in locked room alone. if needed adult diaphers can be used. make sure patient take medications regularly. Laxatives can be given and advise not to cough and stain. avoid loud noise aswell.
