CASE HISTORY: John, an overweight 49-year-old man with a history of diabetes and hypertension is playing soccer. After half an hour of an intense game, he feels severe chest pain that travels to his lower jaw. He is pale, diaphoretic, and short in breath. Upon arrival to the ER, an ECG was taken and the results show the following (note the changes in leads II, III and aVF):
QUESTION: What could be the possible reason for John’s chest pain? Explain your answer based on the clinical information.
What could be the possible reason for John’s chest pain?
Heart failure, it is a condition in which your heart is unable to provide enough blood to the body.The narrowing and blocking of blood vessels caused by high blood pressure increases your risk of developing heart failure.
In this scenario Mr John was physically exerting while playing consequently his blood pressure levels went up and hence the demand of blood in the whole body is increased but due to hypertension his heart is unable to meet the demand and hence it resulted in severe or excruciating chest pain because of narrowing of the coronary arteries.
As per ECG it is evident that the patient is having ventricular fibrillation which is a serious situation that can lead to a heart attack if untreated in time . Also he has every visible signs and symptoms of a heart attack.
He's obese and know diabetic, it put his condition to further risks.
Diabetic patients have an increased risk of developing heart failure because of the abnormal cardiac handling of glucose and free fatty acids in their body .
