Ans)as a nurse I'll identify the health problems of the subpopulations: understanding the problem of the individual.i  will conduct a survey /study of population health carried out by creating health profile...(it include complete health history of the person).i.e. statistical analysis mortality,morbidity and factors influencing health and disease in community.

Secondly,Health assessment :it is a systematic analysis of the health issues facing a population leading to agreed priorities and resources allocation that will improve health reduce problems.

to improve the health of the subpopulation:

Establishes an interpersonal relationship intended to increase or inhance capacity for self care and coping with the community.

Help individuals,families,groups,organizations,and community identify and access necessary resources to prevent or resolve problems.

interventions to improve health of subpopulations:

health promotion: education,counselling,support tools.

Disease prevention:risk assessment,screening,treatment.

Disease management: patient monitoring,treatment,counselling,case coordination.
