Loose stool/ Diarrhoea is watery stools that occure more frequently than usual.

it is usually caused by a virus , or somtimes, contaminated food. Less frequently, it can be a sign of another disorder, such as inflammatory bowel disorder or irritable bowel syndrom.

Nursing assessment of Loose stool

While assessing patient collect the following data in detail

  • Frequency of loose stools
  • Abdominal discomfort/ pain
  • Check for hyper active bowel sensations
  • Stool Route in Laboratory tests
  • check the chances of food poisoning
  • Hydration status
  • Blood test , A complete blood count tect help to indicate What causing diarrea
  • Vital signs
  • Intake and output Chart
  • Colure, texture of stools
  • Inspection, Palpitation, auscultation, percussion in addition to vital signs.
  • examination of full body systems includes cardiovascular or musculoskeletal systems.
  • Check the nutritional status of the patient
  • previous hospital admissions
  • present medications
  • in excisting illness
  • presence of life style disaeses
  • past surgical hystory
  • allergies

You should collect all abouve data and document it ,

if the doctor is not available , you can start to hydrate the patient in order to prevent dehydrate, IV

Ringer Lactate can be started minimum of 50 ml per our.

In form the docotor on call regarding the patient condition ,assessment and Lab report if any , get the doctors order for medication online start with the medications.

Your recommandations to doctor is causes of the diarroea may be due to food poisonig/Stomac flu/Lactose intolerance/ Food allergies/ Giardiasis according to the subjective data as well as your findings.

You can also initiate the doctor to consult the patient immediatly .
