A 6 year old girlis having fever, feeling stiffness on her neck, lethargic for most hours of the day. After asessment she was diagnosed with Meningitis.

Subjective findings:-

1.Confusion

2.lethargy,

3. photophobia

4. headache

Objective findings:

1. Fever

2. Increased ICP (intracranial pressure)

3.Seizures

4. Nuchal rigidity

Actual & potential risk:

1. Acute pain related to headache, fever, nuchal rigidity

2. Risk for infection related to contagious nature of organism

3. impaired physical mobility due to nuchal rigidity & restraing devices

4. Risk for impaired skin integrity related to immobility

5. Risk for injury related to seizure.

6. Activity intolerance related to fatigue.

7. intrrupted family process related to situation
