What is the rationale for having your patient perform mini squats versus tibial on femoral full knee extensions after ACL reconstruction especially if the strength of the repair is in question? Please draw force vectors mft, mfn, etc. and explain. If they had to do full OKC knee extensions how would you do them?

The Anterior cruciate ligament(ACL) is for stabilizing the knee joint while doing kicking or pivoting.As age progresses the ACL ruptures and knee becomes unstable.ACL Reconstruction is the surgical procedure for the condition .It is an arthroscopic procedure in which the ruptured ligament is removed.

  1. The main goal for performing mini squats versus tibial on femoral full knee extension during the stage 1 of post op is for the early strengthening.But aggressive activities may loosen the attachments of the graft,resulting in graft failure.Therefore controlled motionhelps in increasing the bio chemical properties of the graft.As it involves 3 phases in graft ligamentization .They are Incorporation,Revascularization and Graft Healing.

3. .There are two types of knee extensions they are Open Kinetic Chain excercises and Closed Kinetic Chain Excercises.OKC excercises increases anterior tibial translation resulting in strain to the ACL graft.if the OKC excercises are performed within a limited range and time it will be advantageous.there are some minimal differences between OKC leg extension and squatting.
