Explain what informed consent is and how you would use it as a fitness professional with a client? How would you use SOAP notes with a client? Provide specific examples of its use within the field.

Informed consent is a consent given by the client to his/her doctor by which he is giving permission with full consciousness by knowing the risk, benefits, purpose, and all the consequences for a specific intervention, treatment, or clinical trial.
As a fitness professional, I will inform the client regarding the therapies which I am going to give, type of exercise the client need to do, what are the possible health effects after receiving it, what can be the possible outcome, and how the client need to perform during the session.
All the information I need to provide to my client and after that, if the client will agree to receive it after knowing all the possible outcome then only I will move forward.

The SOAP note is used by a health care professional and a physical therapist to assess the client's condition and plan a module of care accordingly.
SOAP note stands for
Subjective data (What patient tells about his condition)
Objective data (What a caregiver observes)
Assessment (In this stage the caregiver further analyze the patient ex. Physical assessment)
Planning (In this stage the caregiver plan the treatment according to patients condition)
Subjective data:-
- First of all, before providing a plan of treatment I need to understand what is the problem of the patient and what condition in which the patient is currently going on.
- The patient data is very much necessary to understand regarding his/her health condition as well as the emotional status.
Example - A client with right5 hemiplegia tells I can not work properly.
Objective data:-
- The physical therapist observes that the client having difficulty in moving the legs properly.
Assessment
- On physical examination the therapist found that the muscle strength of the patient leg is 3/5.
Plan
The physical therapist plan to give passive excecise to patient.
