Transference
Transferance is an unconcious response in which the patient experience feelings and attitude towards the nurse that were originally associated with other significant in their life.It is of two types namely Hostile transferance and Reaction dependent transferance.
Example of transferance;
A pateint named X is diagnosed with Dementia of Lewis bodies ,which had advanced stage that X has incontinent of urine,but not feces.
X was oriented to self : that is X knew about his identity,but not where is lived or about the date.X was able to ambulate with difficulty and needed a wheelchair for lingy distances.X had many parkinsonian features bon medication.X need to taken care for 24 hours with two nurses ,Ms.A ,as the bad one ,he often complainted that Ms.A was mattering him because he thought she was overly strict and dominating .There was no evidence of abuse that anyone could see.The night time aide,Ms.B ,X considered as good one and often called her by his wife's name.
From this Mr.X has negative transferance during daytime and positive transferance during night time.
Countertransference;
Countertransference is the response that is elicited in the recipient that is therapist by the others that is patients unconscious transferance communication.
There are of three types . Reaction of intense love and caring,Reaction of intense disgust or hostility , Reaction of intense anxiety often in response to resistance by patients.
Example;
Ms.Q had a history of repeated treatment episodes for eating disorder, depression and relationship problem.When she moving to University ,the university general practitioner reffered her to the local psychiatric service for treatment.She confined in the young that he was the first doctor to whom she had been able to talk freely and that she told him things,she had never told to the previous doctors.The doctor enjoyed this idealisation and accepted that he had a special relationship with the Patient.
Triggers of counter transferance
That is inability to empathize with the pateint
Depressed feelings during or after the duty.
Carelessness about implementing the contract by being late , running over etc
Feeling anger or impatience because of patients unwillingness to chance .
