It is the responsibility of the coding specialists to correctly code the diagnosis and procedures which is perormed by health care providers.The procedure notes and diagnosis of the condition need to be accurate ,because the coders look up for codes based on the given information.if the information is given wrong it could have a rippling effect n the medical billing.As it is used for medical claims.Inaccurate and contradictory or improperly crosswalked codes are reasons a claim may be denied.All the diagnosis for al the procedures are not necessary.in the coverage policies there are documentation requirements which should include diagnostic test values that must be met,less invasive treatments be attempted before the service is determined to be medically necessary.The diagnossis should not be altered to match with patients code ,especially one listed with medical necessity.The diagnosis code which is submitted must be supported by the medical record.Also advice the provider not to perform the service if the circumstance may deem the service not medically necessary.
