As a paramedic, you were passing by in your car and all of a sudden a vehicle infront of you swerved off the road due to speed and flipped. You exited your car to examine the passengers, unfortunately the driver was extremely injured in his forehead and feet, and the wife was suffered from an injury to her arm and has difficulty breathing. There are also kids crying in the backseat. Please descrive the procedure you would take in detail, with respect to priority.

  • Being a paramedic call 911 ( as he is in his car and not in ambulance)
  • Visually check the car for damage , crush , air bags deployment,steering is intact , car is in ignition state or off, number members trapped inside and extent of injuries
  • Try to verbal contact the persons inside to know their condition.
  • Evaluate CAB while checking the persons inside car ,check for presence of hemorrhage and injuries
  • Provide reassurance to the victims and psychological support
  • Meantime the paramedic team comes, Check for C-spine management , apply cervical collar to the driver and the wife.Provide oxygen masks for the driver , wife and kids .Check whether the kids has sustained any injuries.
  • Check seat position ,function and damage to the interior compartment. They should also identify status air bags and determine if the victims are restrained.
  • Seek for any entrapment of lower limbs of driver and wife
  • Removal of doors or roof to be done.
  • GCS evaluation
  • Carry out disentanglement procedures, take care while using sparkling tools to introduce a way to remove the occupants .
  • The time from disentanglement and extrication should be within 20 minutes .
  • Once the victims are removed they have to be safely secured, driver and wife are to be received in neutral positions , apply splints to immobilze the fractured areas and send them to nearby medical center.The kids should be given psychological support and handover them to retrieval team in hospital
