Role of community health services;
Clinical understand the local community health needs
If clinics shut down, patient will go to the emergency room instead.
Community health services role in the delivery system is likely to increase as previously uninsured individuals gain to access to care. Health clinics can also serve as integrated care provides in the community.
Health clinics already face an increasing demand for services as more uninsured and self paying patient seek care. The demands could eventually create a crisis in some community.
If people can't access care in the community. Many will wait until get bad enough to go to the emergency room. It is mostly important to organisations. The staff is uniquely positioned to understand the community and the social and economic environment that impact patient health care decision .
The ultimate goal is to provide highest quality care in the lowest cost environment.

Create connection between vulnerable population and health care system .Providing informal counselling, health screening and referral
Facilitating health care and social services system navigation. Managing care and care transitions for vulnerable population. Reducing social isolation among patient population .educating health system providers and stakeholders about community health needs advocating for undeserved individual to receive appropriate services. Better understanding between community members and the health and social services system.
Enhanced communication between community members and health provider.
Reduced need for emergency and specialty services.
Improved adherence to health recommendations.
Increased use of health care services .
Organisation with leaders who reported a high percentage of low performance had lower HCSHPS results .a lack of alignment at the senior leadership level negatively affects HCSHPS results. Effective leadership training is most important hospital. High ratings on consistency leadership positivelyaffected HCSHPS results. Standardisation of best practice most important to organisations.
