Three collaberative priority interventions for pressure
injury:
Prevention of pressure injuries is by a multidisciplinary
approach which include :
- Nurses
- Physical therapist
- Nutritionist or dietician
Nurses :
- They do risk assessment of the patient by assessing their skin
daily.
- Provide skin care by keeping them clean and applying
moisturizers if necessary
- They help in repositioning of patient every 2 hrs to prevent
pressure injuries.
- They make use of dynamic support devices and transfer
aids.
- They do daily wound dressings if necessary.
- They diapers frequently in incontinent patients.
Nutritionist:
- Proper diet and hydration are vital for preventing pressure
injuries.
- There is increased need for energy , vitamins A,C and E ,
proteins and zinc .
- They also need aminoacids like glutamine and arginine.
- Proper diet should be planned by the dietician according to the
comorbidities or chronic illness of the patient and he should make
sure that the patient is following it strictly.
Physical therapist :
- Physical therapists gives good training to the care givers and
family members regarding the measures to be taken to prevent
pressure ulcers like repositioning , transfer of patient and type
of beds and surfaces to be used.
- They give good wound care and can do simple wound
debridements(removal of dead tissue).
- They also educate the patient with sttengthening exercises
which prevent formation of pressure ulcers.
