Three Priority assessment of Patient with heart disease:-
1. The patient may have difficulty in breathing due to heart failure, the nurse should assess the breathing pattern(rate, depth, and type) of the patient.
If there is the presence of dyspnoea the nurse should provide a comfortable position, oxygen therapy, and a well-ventilated room.
In this case dyspnoea mainly due to pulmonary edema(deposition of fluid in air space), the nurse should give diuretics to the patient to remove extra fluid from the body.
2. Assess the intensity, type, area, and frequency of chest pain, also assess the relieving factors like rest and nitroglycerin medication.
Limit the physical activity of the patient to reduce contractibility of heart which will ultimately reduce myocardial oxygen demand.
3. Assess the knowledge of the patient regarding diet to prevent further complication in heart disease, and provide health education regarding the dietary pattern in heart disease.
Advise the patent to avoid saturated fat and trans fat diet, and to limit carbohydrates. Encourage the patient to eat unsaturated fat which increases HDL level in the body.
Tell the patient to stick with low sodium diet.
