c)_You are to take the vital signs of a young male in the emergency room. He is on the phone, having a heated conversation. You found him gesturing violently and pacing in the room. What is your nursing approach to accurately measure his vital signs? What about the blood pressure and the heart rate you measured?

a) The general survay information I should collect from the patient are =
i. Personal information like as name, age, sex, time of arival, address as per information.
ii. Problems that he/she is suffering from. Duration, time of onset, what preventive measures he takes at home.
iii. Past medical history likes as any previous hospitalization, blood group, immunization history, psat history of chronic illness.
iv. Personal information likes as type of family, no. Of family members, marital status, relationship with family members, relationship with neighbors.
v. Detail history of illness.
b) The general surya information I shall collect from the client are =
I. some identification data like name, age, sex, date and time of administration, Diagnosis of the client.
ii. A gross information about Complications of the client during admission and complications of the client in present date has to be collect.
c) My approaches will be to measure the vital signs of the client are =
I. At first I would talk to the patient very quietly as possible and tell him not to take the mobile phone with him. Now it is time for taking his blood pressure, temperature, respiratory rate, pulse rate. If he walk so quickly and had a mental stress like then his vital signs will change. BP will increase, pulse and respiratory rate will increase and temperature will also increase so, we can not get proper values of vitals and state of his body.
ii. I shall give him some time to be relax and talk to him to deviate his mind from that topic which he was talking in mobile phone.
iii. After he return to his normal state of mind and body then I shall take the vital signs of the client.
The BP and heat rate will decrease after relaxation therapy.
d) The mose accurate temperature is measured by taking rectal temperature, but this process is not commonly used. The ear temperature (measured by Tympanic thermometer) is another site from where we can get most accurate temperature of body.
