- General appearance -- General appearance
include physical activity, muscle tone, posture and level of
consciousness.
- Vital sign--- Vital sign include pulse
(femoral ) , respiration, temperature and blood pressure.
- Skin --- Assess skin colour, texture, nails
and any rashes.
- Head and neck assessment
- Face and mouth assessment
- Lung sounds and heart sound
- Milestones -- Assess motor skills
- Assess reflexes
2. INTERVENTION TO TREAT PAIN :
♢ Non-pharmacological interventions ---
- Distraction
- Relaxation or imagery
- Music therapy
- Superficial massage breathing techniques
- Heat or cold application
- Positioning and repositioning
♢ Nursing Intervention ---
- Foresee the need for pain relief
- Acknowledge the report immediately
- Assess the pain score
- Get rid of additional stressors or sources of discomfort
- Provide rest to promote relief, sleep and relaxation.
♢ Pharmacological intervention ---
- Acetaminophen and cyclooxygenase given.
3. MALNUTRITION CAUSES OBESITY :
Causes of malnutrition include inappropriate dietary choice, low
income, difficulty obtaining food, various physical and mental
health conditions.
Malnutrition in all its forms include undernutrition, inadequate
vitamin or minerals, overweight and obesity.
Obesity is associated with vitamin -D deficiency. Being obese
can cause a deficiency in vitamin -D, a nutrient that helps the
body absorb calcium which essential for healthy bone.
