The Nursing unit from ER team coordinated a Code Blue call from dialysis unit
The ER (emergency resuscitation team) has designated trained nurses for attending code blue situations in the hospital. The role of ER team is to immediately put best possible efforts to save lives, reach the call within no time, provide CPR or defibrillation to the patient which ever is necessary, intubate if necessary and shift the patient to ICU and provide care circumstantially
The patient was on dialysis routine and collapsed during the procedure, the ER team was called by announcing code blue. The ER Team when reached the patient was asystole. CPR was provided for most possible sequences, but the revival of the patient was not possible. The patient was declared dead.
The consequence of the incident report suggested the patient suffered major cardiac arrest during the dialysis procedure. There were certain facts that needed to be taken into consideration in the report a. the patient was 70 year old b. the patient had history of cardiac ailments along with hypertension and diabetes, c. the dialysis poses a risk for the patient with such history and extreme situations d. the patients informed consent and risk assessment forms were duly signed and the relatives understood the risks of the treatment given to the patient,
Under the current circumstances the role of nurse leader is to take of note of the incident report and follow it and understand the various steps followed by the nurse in charge in the dialysis unit as well as the steps followed by the ER team.
The nurse leader prepares a detailed summary of death and provide any additional information needed by the relatives to claim the rights of the patient.
The nurse leader observes any flaws on that need to be improved
The nurse leader encourages the staff to put forth their duties and regularly plans for mock drills to keep the teams on their toes
The nurse leader also can plan for educational programs that require to be delivered to the required teams to keep the knowledge updated
The nurse leader bridges the gap between management and the nursing team
The nurse leader shall be the face of the nursing care provided at the behest of the nurses
