Where and how can you find information to ensure good case management practice?
Case managers need to assess the client's cultural believes,interest,wishes,needs and values to ensure great service to the client .For that they need to evaluate and understand care options available to them.They have to agree on their care goals and priorities.We have to determine what is best to meet their needs.Institute action to achieve their goals and meet their interests or expectations.How case managers come in contact with clients and their support systems varies based on your practice settings.
Collecting from payor-Direct contact via the telephone can help the case management team to collect information about the client or upon referrel from other professionals working for the payor organisations such as medical director,a claims adjuster,a clerical person or a quality improvement specialist.
Acute care setting-based case manager may impliment the case management process for a client after referal from any of the healthcare team members including the physician,primary nurse,social worker consultant and specialist.
Direct hospital visit
In some organisations case managers may vistit every new admission and conduct high level review of client situation for the purpose of identifying whether the client would benefit from the case management surveys.
Community care settings-Community care settings may implement the case management process for a client after referel form the primary care provider or speciality care provider.In some instances the case manager can initiate the process base on a request from support system on regulatory obligations such as helth home requirement for medicare.
Vocational rehabilitation- Vocational rehabilitation case manager may impliment the case management process for a client after referel from accute or community based case manager.
Employer-employer may refer a client[ ,especially cases like injury during job] for vocational rehabilitation as part of the return to work plan of care
