CASE MANAGEMENT

Case management is a process within the healthcare setting which strives to achieve specific goals by handling cases from begining to the end under coordination of a case manager.This process will include multiple team contribution to attain the goal.

THE SIX COMPONENTS OF CASE MANAGEMENT

There are variety of components wchich is included in CM the main 6 are listed below.

  1. ASSESSMENT
  2. PLANNING
  3. FACILITATION
  4. CARE COORDINATION
  5. EVALUATION
  6. ADVOCACY FOR OPTIONS.

In that 3 scenarios a case maneger or case management team will work collabratily to give possible care.

1.When a cancer patient is taken for biopsy, the patient would have been checked several times for any chance of collapse during biopsy.However, the management team will work more efficiently during critical situations with all facilitated preparedness ,and preventing further comblication,response to the situation accurately and then assess the patient and amke a easy way to recovery.

2.LIVER TRANSPLANT PATIENT ADMITTED WITH BOWEL BLOCKAGE.When it comes to such patient during his surgery itself the case management team would have been assessed him fully and this patient dont have to get exposed with all the initial procedure to get a sterile condition.As bowel bloackage is an anticipated complication of liver transplant the CM team will be regularily contacting the patient to identify any early sypmtoms of complication and to seek best medical support.In another way the team will be having whole history of patient which helps to kick start the process than lagging for history collection and other procedure.The team will guide the patient about the present condition and speed up with the interventions.

3.  Collabrative care for ,management of depressive disorder is a multicomponent,the healthcare system along with case management system will provide comprehensive care with surgical healthcare team with mental health speacialists.A depression pateint will be taken care for his insecurity with 3rd surgery by giving possible health education and counselling .The drugs to reduce the mental conflit will start prior to the surgery and the team will assure to walk along the patient through all stages of care.
