A tiny number of people have often come to think they are born into the wrong gender. Gender dysphoria, as it is known, defines the depression they feel as a result of the mismatch between the biological identity reported on their birth certificate and the gender they associate with. In the past century, medical care was available to enable transgender people to permanently alter their identity and gender by hormone therapy and surgery, from one identity to the other. However, it has been proposed more recently that society should accept people who identify as transgender even though they do not wish for any medical operation to alter their biological sex. In other words, a person should be able to define himself as the opposite gender of their biological sex solely by means of personal choice and society should be obligated of view them in the the same manner as others who share their 'different' gender but are born with that gender's biological sex. It is the ramifications of this argument, most recently in sport, that create a heated and sometimes deeply acrimonious debate. The International Olympic Committee had changed its rules back in 2003 to require transgender athletes to participate at the Olympic Games. But it is only recently that the IOC lowered its requirement that only those transgender athletes who had undergone surgeries should compete in order to change their physical appearance to suit their gender. Today it calls for transgender athletes who have undergone hormone therapy for twelve months but not surgery. For reality, this focuses mainly on athletes born as males who have converted to become females, the hormone treatment needed to reduce their testosterone levels to the levels seen in born girls female athletes. The International Olympic Committee in 2004, after evaluating the facts, allowed transgender athletes to compete in the gender to which they had been transformed. The athletes are expected to perform sexual reassignment surgery and undergo at least two years of hormone therapy (female-to-male testosterone and male-to-female testosterone suppression) to participate.
