Put together a data collection plan for errors generated in a hospital. This may be a group activity. You may choose either medical or administrative errors or both based on your experience.

Data Collection Plan for administrative errors-
The first step in my plan would Be identify the question or problem we want to answer-
This is very important step of our data collection because if we do not know for what we want to collect and what is the reason to collect data then it is very weak start. Our data must be relevant to our research.
for example- our question is to why there is increase in medical error reporting ? or Why medical reports take too much time? or why recording of medical information of patient is not relevant and organized properly?
The second step in my plan would be Type or kind of data available in our hospital-
There are two types of data which can be present in our hospital. primary data which we can collect from administrative staff of hospital and secondary data which involved case studies, past records, reports, online research etc.
Third step in my plan would be deciding how much data i needed-
Data is something which never be less in any research. You can spent years to collect data but we want to set amount that will be sufficient to answer our research. We want to get enough data so what we can see patterns and trends.
The fourth step of my plan would be methods to collect data-
In this step, i would use survey where i would send questions to staff in hospital.I will do face to face interview questions with supervisor and manager of hospital . I would observe the working of employees in administration department.For secondary research, i would use database of hospital, reports, newspaper and online library to complement my research.
The fift step of my plan would be How to Display-
The data collection plan also explains the format in which the collected data needs to be displayed to the Six Sigma team. Most probably a graphical method is used because it is intuitively easier to use.
