1)List the steps for safely administering an ophthalmic medication:
- Eye is a vital part of visual system. It helps to convert light into meaningful images. Eye drops are used to treat eye infections, to do ophthalmic examination,to relive irritation,to treat dry eye syndrome. The eye drops must administer at correct time,correct dosage,to right person,and right eye.
- Position patient in a comfortable position sitting or lying with supporting head.
- Wash hands before initiating the procedure.
- Clean gloves can be used for instilling eye drops. To administer eye drops to a post operative patients sterile gloves can be used. Shake eye drops to mix drugs in correct proportion.
- Lower the eyelid and ask patient to look upward and instill the drop into the space.
- Encourage patient to look up in order to prevent deposit of eye drops in the cornea.
- Instill only one drop to prevent overflow of eye drops.
Procedure to apply eye ointment:
- Wash hands before applying ointment in order to prevent cross infection.
- Sterile gloves are used to apply eye ointment.
- Apply thin ribbon of the ointment into the lower fornix of the eyes.
- Incase of excess spilling of ointment wipe it out.
- Wash hands after the procedure and record the procedure.
2) A nurse is caring for a client that exhibits signs and symptoms of aspiration during their enteral feeding. What is a priority intervention the nurse should carry out?
Enteral feeding is the process of administering food into the gastrointestinal tract. Tube feeding is indicated for the client who cant eat safely on their own, oral cancer,facial fracture, postoperative, malnourishment,stroke,facial paralysis. Different types of enteral feeding are : Nasogastric feeding, orogastric tube, gastrostomy,jejunostomy.
During feeding if the patient exhibits signs and symptoms of aspiration the immediate action of the nurse is to stop feeding as further introduction of fluid will results in atelectasis. Then put the patient in semi recumbent position to prevent aspiration, suction the oral airways to remove aspirated fluid, oxygen administered if there is marked reduction in saturation. Once early intervention is done the position of the Ryles tube is checked by radiographic examination.
