Deficient Knowledge r/t insufficient information AEB mother being upset and expressing that she had not heard anything about cerebral palsy before. -Measurable Patient Outcome Mother will be able to express an understanding of cerebral palsy, the need for ongoing treatment, and supportive resources that may be available prior to being discharged. -Nursing Intervention Consider the health literacy and the readiness to learn for all clients and caregivers (Ladwig, Ackley, Makic, 2017, pg. 493). -Rationale The mother has expressed that she had not heard anything about cerebral palsy in the past. The RN needs to take this into consideration when educating the mother on this condition to assist her in reaching a better understanding since she has no previous knowledge. The RN should refrain from using complex medical terms that the mother may not understand. Doing so, can cause a barrier in learning and the mother leaving the facility with unanswered questions. The mother’s emotional state may also become a barrier in the learning process. Along with the RN providing education appropriate for the mother’s understanding, the RN should also provide information on available resources and support groups for families dealing with similar circumstances. This will be helpful for the mother since her readiness to learn may improve after her initial learning of her child’s diagnosis and improve her ability to cope more effectively. -Nursing Intervention Consider the context, timing, and order of how information is presented (Ladwig, Ackley, Makic, 2017, pg. 493). -Rationale The mother is upset upon hearing this diagnosis for her child. Prior to attempting to educate the mother on her child’s condition and treatment needed, the RN needs to take the mother’s feelings into consideration to promote a better patient outcome. The RN should provide the mother privacy, allow her to express her concerns openly, offer emotional support, answer any questions she may have honestly, and show empathy towards her feelings. These actions if demonstrated by the RN will help to build a therapeutic relationship that will be beneficial during the teaching process. -Interprofessional Health Care Team Member to include Physical therapy services should be included in the plan of care for this child. -Rationale The physical therapist can educate the mother on exercises to preform for her child in attempt to prevent further decline in the patient’s motor function. “Preventing contractures is vitally important to maintain motor function. To help avoid these, teach parents that the passive exercises and games their child has been prescribed are an important part of their child’s therapy and must be done consistently each day” (Pillitteri, 2014, pg. 1444).

Yes, It is necessary for Registered Nurse (RN) to assess the mother's understanding level in order to provide best possible way or simple language about the child's disease condition knowing this will create a grief process for mother & all family members.
So RN must see to it that she handles the grief process effectively by problem solving appropriate, reinforcing them, building in confidence level. As going through this phase is tough as emotionally it gets hurt.
But RN must develop a therapeutic approach to get back the mother's self confidence level to manage the child's condition.
Also there are various therapies which are needed in management of child's condition.
• Physical therapy
• Occupational therapy
• Speech therapy
• Assistive technology: walker, crutches,etc
• Mediations
• Surgery
So here one of the interprofessional says that physical therapy is yes as it is essential, helps strengthen the muscles to avoid weakness and deterioration, helps in gaining gait so as to have Independence in life.
Also exercise and games play important part in child's therapy as it helps in physical, mental activities indirectly facilitating motor function. Participating child in such activities will improve child's condition effectively rather than deteriorating over the time.
