The second reading is close to the first reading, what questions would you ask your patient? After gathering subjective data from the patient, what is your next action?
1. After receiving report from outgoing shift ,make a quick visit to each patient and check for any emergencies that need to be attended . If everything is fine then start with routine care.
Check the medication record for any changes in the medication regimen.
Check for any updates in the patient's treatment plan .
Check the records for lab test and diagnostic procedures that were ordered from the time you left the unit.
2. Firstly informed the patient about importance of this and take a quick assessment about the tiredness. If the patient still refusing the assessment then let the patient to rest.
3.a Recheck the B.P again and inform to the doctor.
b. Ask the patient about family history of B.P.
Ask about diet .
Ask the patient about history of alcoholism .
C. Prepare a nursing care plan for this patient - educate the patient about how to manage hypertension, encourage sodium restricted diet , implement regular physical activity
