The patient those who are suffering with chronic conditions the informatics includes patient participation, public health, patient outcome.

  1. Patient participation: Encourage the patient to participate in the community activities such as maintaining the personal hygiene and environmental hygiene and administration of medication in correct time and take the advise from the Health care system.
  2. Public health: Here public health also play an important role in the informatics it will rendering the services to the patient those who are suffering with chronic disease and as a health care professional we need to provide therapeutic regimen for the patient.
  3. Best patient outcome: After taking treatment from the community for the Cronin disease through the informatics it will give positive feedback in order to improve best outcome of the patient. The health of the patient is also cure and he can able to follow the instructions by the health care settings.
