For a focussed assessment we should consider following things.

Q. 25yr old patient with c/o left sided earache for 1 week.

HISTORY TAKING

History taking of present complaint.(the reason for which patient has come to hospital or to whomsoever voncerned)

SOCRATES short acronym for remembering each point for askinh.

S SITE-  Where is the pain occurring?

O ONSET- Whether the pain was gradual arising or immediate/sudden?

C CHARACTER - Whether the painwas sharp , like pin pinting , burning etc.?

R RADIATION- Did the pain radiate to other site than the original site?

A ASSOCIATED WITH - Is the pain associated with any other thing like pus etc.?

T TIME PERIOD - What is the duration from which pain is occurring?

E EXACERBATING FACTORS( AGGREVAITING)/ RELIEVING FACTORS- What are the factor that aggreviate or the relieve the pain ?

S SEVERITY - We can ask the patient to rate the severity of pain from 1-10.

Physical examination

It can also be done to ensure the complete diagnosis of the disease.

The concerned body area is examimed here ear.

Otoscope can be used to view the eardrum and the earcanal.

Hearing tests include WEBER'S TEST AND RINNE'S TEST.

---THE SAME POINTS THAT ARE MENTIONED ABOVE IN HISTORY TAKING SHOULD BE CONSIDERED IN SECOND CASE ALSO.

These are the basic points that are asked in diagnosis of every complaint /disease.

In physical examination, here the examination of the paritid swelling we will ask the patient to clench his teeths to that massester is palpable because parotid is best felt below the massester muscle, and examine the right side.

We can note the color , size of the swelling.
