1)the community health nurse plays an important role in helping the elderly maintain an independent life in the community for as long as possible.
2)it does this by emphasizing preventive geriatric medicine,provision of supportive and care services and ensuring emotional and physical support carers.
3)integrate advanced knowledge and experience in delivering safe , effective quality care to geriatric clients in primary care.
4) demonstrate competence in managing the health /illness status of geriatric clients in primary care.
5) monitor and ensuring quality healthcare for geriatric clients in primary care.
6) incorporate an understanding of trends in aging in planning and providing primary health care for clients.

Physical barriers,resouses:these people may want to get treatment,and may be able to access a hospital or clinic,but they will not have the means by which to pay for health care services.inability of clinic or hospital to gather enough supplies to deliver proper medical treatment.
Cultural and geographical barriers:some cultures place great importance on religion and hold belief that everything is on God's hands.
C) urban vs rural areas affects home care services:
Rural vs urban:
Home health services are not readily available in rural areas,nor are they used to their full capacity.reasons are:
1) regulatory barriers in medicare,medicaid,and state certificate of need requirements.
2)lack of knowledge about home health services and eligibility in communities,particurlarly for care coordination.
3) inadequate reimbursement,particularly in rural areas where longer travel times and costs are not taken into account in reimbursement rates.
4)rural home health user's receive on average three more visits than their urban counterparts,with many more skilled nursing and home health aide visits.
5)rural enrollees tend to be older than their urban counterparts and are more likely to be male and white.
6) rural episodes of care appear to be consistently longer in duration than urban episodes.
