Identify and define the Long Term Care organizational hierarchy, and explain each role.
Long term care brings a wide range of services for persons who are dependent on help with basic activities of daily living.It is composed of several different teams
The Administrative team,Coordinating team,Ancillary team,Support services team,Core team and Contigency team.
The Administrative team which consist of the medical director,the nursing director,and the administrator which includes overall leadership and has 24 hour accountability for the overall function and management of the long term care.They provides the faramework and guidance that ensure each team understand its role and responsibility and has access to the necessary resources to be successful.
The Coordinating Team is responsible for day to day operational management ,resource management for core teams and coordination of functions.They provide policy-level guidance,triage all emerging events and prioritize decision making.They facilitate core team actions and outcomes by collaborating it with the administrative team and the Ancillary servicesteam to assign priorities.It consist of the nursing supervisor,unit secretary and dietitian.
The Ancillary team coordinates direct,task-specific,time limited care to residents.They are a service delivery team whose mission is to support the core team and functions independently.It includes rehabilitation therapy staff,social services,and therapeutic recreation staff.
The support services team provides indirect,task-specific services to the whole nursing home.it consist of nursing homes emergency response team,care planning team and quality improvement teams.
The Core team consists of team leaders and members who are involved in the direct care of residents.it consists of nurses such as charge nurses ,medication nurse,nursing assistants,restorative aides,attending physicians and APRN
The Contigency team provides time-limited assistance for specific or emergent events.
