Failing to communicate important information , especially when it comes to a medical condition can have very bad consequences . Failing to communicate a change in a patient's condition is just once scenario that can result in a gap in the care process that could potentially cause irreparable damage or even death in a medical setting .

Nurses and mid level professional , such as physicians Assistants and Nurse practitioners are highly involved in the patient care ., but there ability to follow the directions and to only treat their patient to the extent that they are licensed to do so makes them vulnerable to being named in a medical malpractice suit,,

A shared an experience of a malpractices case where the error of a nurse played a large role in the treatment of a patient in the hospital .. There is the patient suffered by congested heart failure , and he is admitted from a last week , It was the time of mid - night when the patient can't breath properly ., he hyperventilates and the blood pressure goes rapidly high . then the night shift nurses , physicisian rapidly run towardds the patient , it's time of emergency condition all action are demand  in the appropriate manner . immediately patient experience seizures , and cardiac arrest , doctor order the one nurse to do CPR , and other one to the administering the atropine , but that nurse is buzy in doing ECG . she hands the ECG machine over another nurse and run away to take medication . She fails to remember the name of medication atropine instead she take potassium cholride vial and start to load in the syringe . when during admintration doctor again ask to confirmation that it is atropine , she replied no , its the potassium chloride . then doctors and the all nurses staff badly react on her and scold on her doctor , loudly said ''that did going top mad , are you waht to kill the patient '' , so, such communication failure is become the lethal at this moment , that paient could be died immediately if this potassium chloride loaded injection administer in it .
