Medication error -A medication error is any preventable event that may cause or lead to inappropriate medication use or patient harm while the medication is int the control of the health care professional,patient or consumer.

-wrong preparation

While preparing training plan the following guidelines to be followed. there are 10 strategies for preventing medication errors

*ensure the ten rights of medication administration . check right dosage, drug,patient . route ,frequency before administering drugs  

.*follow proper medication reconciliation procedures

* double check -or even -triple check procedures- it is a process where by another nurse onn the same shift or an incoming shift reviews all new orders to ensure each patients order is noted and transcribed correctly on the physician order and the medication record.

* have the physician or another nurse read it back

* consider using a name alert- some instiuitions use name alert to prevent similar sounding patient names from potential medication mix up
