The Pennsylvania psychiatric institute in Harrisburg reduced its 30 day readmission rate from 20% in 2013 to 10.4% in 2015 this is considered as one of best program by implementing a discharge program modelled after AHRQ’s Re –Engineered Discharge (RED) toolkit. According to the research it was identified as its previous program of is 30-day readmission rate than three other acute care facilities in the region.

The institute main objective in serving the psychiatric people are

  1. By implementing a reduce 30 day readmission rate among its high risk population
  2. The worked to improve the quality of service and care by addressing the service gaps
  3. It organized patient care by improving patience adherence and hospital care instructions
  4. It also implemented reduce penalties for excessive readmissions

The Agency for Healthcare Research and Quality (AHRQ) developed a model on which the READY program was based. The program basically follows key steps to implement the better service programs that reduce the mental illness outcomes. Basically the half percentage was identified which is called as one of the most success program.

Patient started giving ratings to the current program by rating 9.21 out of 10 during 2013 to 2015.

Apart from this to meet the objectives, the institute developed a behavioural health support initiative called the discharge READY program, modelled AHRQ’s RED toolkit.

How to monitor implementation and outcomes

These are the programs that are followed by calling to the mental illness patients for better follow-up after their discharge the nurses’ calls the patients during their five week program. This program begin 48 to 72 hours once the patient reach their home. In this program Nurse Monitors and reviews their conditions and provide information and support based on the conditions. This is called the patient and nurse development action plan for each week.

These programs are considered as success programs which brings down the percent of change in the patients.

Apart from this follow the other effective programs

  • Keep an eye to every patient to monitor their changes in the day by day activates
  • Treat them as regular patients to avoid the difference between normal and mental patients. Which brings positive results in their mind and tend to learn to be normal.
  • More exercises and other physical activities needs to perform for better improvement.
  • Group discussion and group involvement also make them very happy to come out of that state .This change the environment and atmosphere which bring good results.
  • Based on their interest take them to interest areas where they are interest to spend time and enjoy nature.
  • Every time it’s important to communicate with them more and more to know their conditions and health improvements. Through communication more success results can be carried for more and more health results.
