Please answer in short and quick
a) Dug ABC is an orally administered anti-asthmatic drug, which is
a substrate of CYP1A2. It is given to a patient who is on Drug XYZ,
which is known to be a CYP1A2 inducer. Will it be appropriate for
the patient to take Drugs ABC and XYZ together? Explain your
answer
b) Drug ABC undergoes glomerular filtration and proximal tubule secretion. Explain what will happen if a patient has reduced blood flow to the kidneys.
Ans:- concept :- cytochrome p 450 group of enzymes are mainly involve in deactivations and metabolism of drugs.
when a substance induces it's activity then rate of metabolism of drugs increases and drugs become inactive very fastly leading to decreaeased action of the drug.
now :- if drug ABC and XYZ are given simultaneously then drug xyz will increase the metabolism of drug ABC by ok inducing the enzyme cyp1A2. Leading to decrease bioavailability of drug ABC and loss of its action.
Hence overall effect of drug ABC will decrease in the body.
CYP1A2 plays an important role in metabolism of drugs like clozapine, olanzapine, fluvoxamine, haloperidol, theophylline;
Ans b:-
Concept :- the main function of kidney is to excreate drugs out of the body.
If blood flow to kidney is decreased then excreation of drug out of the body wil also decreases leading to increase level of drug in the body , leading to over dose toxicity and various side effects.
