Tully is a 75-year-old female who has an intellectual disability.
She enjoys art, reading and music. She is not overly social and tends to spend time alone privately completing recreational activities. She has been a smoker for 40 years and usually smokes up to 8—10 cigarettes a day. She is able to go for walks for short periods at a time only. Tully is sensitive to loud noises in the environment, which causes her to become quite anxious and stressed. Being anxious or stressed causes Tully to smoke more frequently through the day. Tully’s father passed away from lung cancer due to smoking when she was younger, so one of her goals is to cut back on smoking. She is hoping to smoke only 2 cigarettes per day. Smoking habit can be removed by the means of pharmacological management as well as from non pharmacological management. We will see it one by one:
a) pharmacological management - i.nicotine replacement therapy- present in form of nasal spray , transdermal patches, gum patches etc.
ii. Oral bupropione- 150 mg qd/3days after that 150 mg bid.
iii. Vareciline- is an agonist of acetylcholine receptor, proves efficient for cessation of smoking.
b) non pharmacological management- i.pulmonary rehabilitation - in this particular exercise is recommended to patient like normal walk, slow breathing with nose and exhaling with pursed lips, light weight lifting and performing arm curl, knee extension , under water aerobics exercise prooves to be effective . And many more . While exercise patient should keep in mind that soft and slow breathing should be performed.
All these will help in cessation of smoking.
