Case1:

A bifid right fourth rib around the costochondral joint with bifurcation point at fourth rib. The space between the upper and lower division is called bifid space which looks round and filled with intercostal muscles and fasciae. The sternum was found articulated with the cartilaginous divisions. The breadth of the third and fifth ribs is larger than upper and lower divisions with widen intercostal space between the lower division and fifth rib. Calcified cortex was found around the third, fourth, and fifth ribs to their sternal ends. The third anterior intercostal artery was the main artery which supplies the blood to the bifid space. The other parts of the intercostal muscles were enriched by posterior intercostal artery that branched from the thoracic descending aorta.

Case 2:

A 65 year old male had a bifid right fourth rib around the costochondral joint with the triangular bifid space. In contrast to case 1, it was reported to have narrowed upper intercostal space and widened lower intercostal space. It also observed with calcified cortex around the third, fourth, and fifth ribs to their sternal ends. The vascular supply is predominant by the posterior intercostal artery to the parts of intercostal muscles other than bifid space which is supplied by anterior intercostal artery.

Case 3:

A bifurcation on bony portion with a bifid right fourth rib in the 60 year old male. Here, the bifid space was round and large with narrowed upper intercostal space which almost in contact. The lower intercostal space was relatively lower due to the larger bifid rib and cartilage. There is no calcification in the cartilage. The blood supply is as similar to previous cases. The costal groove and lower division was found with the presence of intercostal nerve of 4th thoracic nerve and the other was found along the superior margin of the fourth rib which innervated the bifid space and was branched from the third intercostal nerve.
