For this project you should use the 5 wishes form provided to you. Complete this form either for yourself or interview a loved one to complete the form for him/her.

Once the form is complete write a brief reflective paper on what you learned through this experience. Include if you discovered anything new or changed your mind about anything through the process.

This paper should be at least 2 pages double spaced but no longer than 5 pages double spaced.

Only this reflective paper needs to be turned in.

Five wishes -Advanced directive projects--Five wishes is a foundation which takes care of the old and sick people. It is a form with attestation from the witness and attorney regarding the well- being and insurance of the health of old persons who are not cared by their own progeny or offsprings. They are kept in particular environments with all the basic health facilities needed for their weel being. These agencies are repsonsible for the well being of the old people who lack support from the friends, offsprings and relatives It incudes-

  • the care of the patient.
  • diagnosis of the mental diseases.
  • friendly behavior by the caretakers.
  • regular visits by the relatives, friends and offspring of the person in the family.
  • nourishment of the skin.
  • regular exercises.
  • check up of the heart rate and ciculatory system.
  • proper facilities with emotional well being and happiness.
  • spiritual happiness and elevation with exaltation.
  • complete dominance with supremacy and independence in life.
  • inner happiness about living with same age group old people where they do not feel isolated .
  • total independence of life.
  • complete healthcare investment with checkup facilities.
  • ambient environment with lack of depression, stress and sufferings in life.
  • no responsibilities and no transfer of will to the offpsrings leading to satisfied life in old age with payments for living compensated from their friends, offsprings and relatives.
  • regular visits by well qualified team of doctors and nurses .
