1) The charge from an ionization chamber is converted to dose in water by applying a potential difference between the electrodes to create an electric field in the filled gas. Now when gas is ionized by incident ionizing radiation, ion pairs are created and the resultant positive ions and associated electrons move to the electrodes of the opposite polarity. Now, each ion pair creates deposits or remove a small electric charge and they get accumulated to form dose in water.
B) In the case of radiation equilibrium with incident radiation photons become equal to formation of dose, then only dose becomes equal to collision kerma.
