Order: Tobramycin sulfate 228 mg added to 500ml Normal Saline IV bag for infusion IV over one hour, t.i.d.
Supply: Nebcin (Tobramycin sulfate) 40 mg/ml.
How much Tobramycin do you add to the Normal Saline?
Use the standard formula
Desired(D) *Vehicle(V) / On Hand (H)
Desired dose 228 mg
On Hand 40 mg per ml
Vehicle = 1 ml
Devide 228 mg ( Desired)with 40 mg ( drug on hand) to get 5.7
Multiply 5.7 with 1ml ( Vehicle) to get 5.7 ml
Computation :
228/40*1= 5.7 *1 ml = 5.7 ml
