resources and peer group support help the patient to adhere to the
treatment. get community involvement to meet the needs of the
patients. make sure that chronic ill patients are supported well,
involved and they are active. it is important to provide high
quality and safe care. evidence-based care provides optimal health
and manages their health condition. regular follow-up with
culturally competent care with patient preference allows the
patients to get involved in patient care. shared information and

HIV and chronic illness patients need patient-centered care,
