The definition of heart failure is failure of heart to contract effectively to pump adequate amount of blood for systemic circulation to meet metabolic needs. If ventricle is fail to contract( hypokinetic) stroke volume and cardiac output reduce. The aim of management is enhance ventricular contraction to achieve opimum level of cardiac output.
Dopamine is a vasopressor. It is a catecholamine ,acts on alpha1 and beta1 adrenergic receptors. It increases systemic vascular resistence results increasing mean arterial pressure and organ perfusion.
Inotropics acts on the myocardial cells.Thus increase cardiac contractility leads to improvement in mean arterial pressure and organ perfusion. Also affect heart rate and increase peripheral vascular resistance.
Diuretics help to decrease fluid overload by increasing urine output. When fluid load is decreased and the peripheral vascular structure and myocardial cell contraction is effectively supported with dopamine and inotropes in cardiac failure, which will facilitate heart to pump easier to eject adequate cardiac output. Same time peripheral vascular resistance enhance venous return.
