Hemodialysis improves insulin sensitivity. Currently, there is no recommendation for the adjustment of insulin dose on dialysis day and nondialysis day in diabetic patients. This study was undertaken to determine the variations in insulin requirement based on blood glucose levels on dialysis days and nondialysis days in type 2 diabetic patients with chronic kidney disease on maintenance hemodialysis. Twenty‑eight diabetic patients on hemodialysis were recruited into the study Capillary blood glucose levels, just before dialysis and 2 h after dialysis, were checked and compared with fasting and postprandial glucose levels on–off dialysis

days. There was significant (35.8%) decrease in blood glucose levels 2 h after dialysis in comparison to predialysis levels (from mean levelof 258–165 mg/dl). The decrease in the blood glucose levels from predialysis level to 2 h postdialysis level was statistically significant (P < 0.001).

What is the design of this study? Provide detailed explanation.

Expert Solution

  • This study has a case control study design. This particular study design mainly focuses on one particular group of individuals and the group of people are checked for their experiences and then the odds of having a particular outcome are measured. These studies are mainly conducted to learn the diseases and the outcomes.
  • In this study the group chosen is type 2 diabetic patients with chronic kidney disease on maintenance hemodialysis. The study was done to determine the variations in the insulin requirement based on the blood glucose levels on dialysis days and non-dialysis days.
  • In study methodology, twenty-eight diabetic patients on hemodialysis were taken for the blood glucose test before dialysis and 2 hours after dialysis, and were checked for fasting and postprandial blood glucose levels on dialysis and these results were compared with off dialysis days fasting and postprandial blood glucose results.
  • The results obtained were,a significant decrease in blood glucose level was found 2 hours after dialysis in comparison to predialysis results. That is 35.8% decrease. The decrease in the blood glucose levels from predialysis level to 2 h postdialysis level was statistically significant (P 0.001).This means that there are very less chances for the results to have errors.Hence the results obtained here are highly significant.
