communication is used in every steps as part of management of care process in our organization. Effective communication is used during screening, assessing, stratifying Risk, planning, implementing care, following-up, transitioning (transitional care), communicating post transition, and evaluating.
Communication is pivotal to health care outcomes, which includes patient safety and patient satisfaction. Communication reduces the chances of a breakdown in continuity of care, builds relationships and understanding.
