Effective communication is used during screening, assessing, stratifying Risk, planning, implementing care, following-up, transitioning (transitional care), communicating post transition, and evaluating.

Communication is pivotal to health care outcomes, which includes patient safety and patient satisfaction. Communication reduces the chances of a breakdown in continuity of care, builds relationships and understanding.

When health care professionals are not communicating effectively, patient safety is at risk for several reasons like lack of critical information, misinterpretation of information, unclear orders over the telephone etc. which can creates situations where medical errors can occur.
