Risk for self harm related to feeling of hopelessness due to psychiatric disorders.

patient will refrain from attempting suicide.
patient will remain safe while in the hospital with the aid of nursing intervention
patient will join with family in crisis
patient will identify atleast one goal for future.
patient will state that he or she wants to live.
patient will name atleast one acceptable alternative to his situation.
Intervention
Arrange for the client to stay with family and friends.
Encourage the client to avoid decision making during the time of crisis.
Encourage the client to talk freely about feelings.
Weapons and pills are removed by the nurse
Implimentation
Relieved isolation and provided safety and comfort
During crisis situation patients are unable to think clearly, alternatives can be considered.
Given client , a support to dealing with strong emotions and gaining a sense of control over their lives.
Provided safe environment, free from harmful things from clients.
