Using the patient profile provided, develop and post an education plan for the patient based on their current health status. Include 2 short-term and 1 long-term goals for the patient and provide detailed education to the patient on how to meet these goals and maintain optimum health. Include information on follow-up care as well.
A.R. is a 26-year-old male with a history of IV drug use. He has been clean and sober for 2 years. He was diagnosed as HIV positive 9 months ago and comes to the clinic for a check-up. He states he has difficulty remembering to take his medications consistently since beginning a new job working nights. His current CD4+T cell count is 850 cells/μL. His CD4+T count at his checkup 3 months ago was 1100 cells/μL. He reports good health and denies any recent illness or infection. He reports that he recently began dating someone 1 month ago who is HIV negative.
The main aim is to prevent HIV transmission and to support of affected person. It is vital that HIV counseling should have these dual aims because the spread of HIV can be prevented by changes in behavior. Determine whether the lifestyle of an individual places him at risk.. Working with an individual so that he or she understands the risks
Helping to identify the meaning of high risk behaviour. Working with the individuals to achieve and sustain behavior change.individual ,relationship and family counseling to prevent and reduce psychological morbidity associated with HIV infection and disease.
This man as dating with HIV negative girl should be counselled about the risk of spread of infection. And even the girl should be counselled. Advocate use of contraceptive method especially barrier method should be followed to prevent transmission of disease and to progress a disease free relation.
Patient should be given moral and psychological support and our main aim is to prevent worsening of disease and prevent further spread. Risk of IV drug abuse should be well educated.
