a. Examples of sentinel event:

sentinel event is an abrupt untoward event that occurs due to a dysfunctioning of the structure and processes in a health care system results in the disability or death of a patient.

Examples: A patient admitted for undergoing a cataract surgery of left eye has finally undergone surgery of the right eye.This is a sentinel event due to wrong site surgery.

Examples: An elderly patient who was admitted for the treatment of diarrheal disease had a fall in the toilet due to lowered side rails and slippery floor secondary to negligence from the nurse of not attending frequently and the house keeping staff. This fall resulted in fracture of the little finger of left arm of patient.
