New Zealand has achieved universal health coverage through a mostly publicly funded, regionally administered delivery system.sevices coverd include inpatient ,outpatient, mental health, and long- term care as well as prescription drugs. General taxes finance most services.the national government sets an annual budget and benefit package .District health boards are charged with planning ,purchasing, and providing health services at the local level.patients owe copayment on some services and products, but no deductibles.Approximately one- third of the population has private insurance to help pay for noncovered services and copayments.
Public hospitals receive a budget from their owners, the district heath boards, based on historic utilization patterns, population needs projection, and government goals in areas such as elective surgery. The budget include the cost of health professional and other staff, all of whom are salaried.within a public hospital, the budget tends to be allocated to various inpatient services are funded regardless of case mix.
A proportion of district heath board funding for elective surgery is held by the ministry of health, and paym are made on delivery of surgery.
The health care sector in the united states consist of an array of clinicians, hospital and other health care facilities, insurance plans, and purchaser of health care services, all operating in various configurations of groups, networks, and independent practices.some are based in the public sector; other operate in the private sector as either for - profit or non- for - profit entities. The health Care sector also includes regulators, some voluntary and others governmental.Although these various individuals and organization are generally referred to collectively as the health care delivery system the phrase suggests an order, integration, and accountability that do not exit.communication, collaboration or system planning among these various entities is limited and is almost incidental to their operations. For convenience, how ever, the committee uses the common terminology of health care delivery system.
The US health care delivery system has long been characterized as complex, fragmented, costly and with significant variation in quality of Care...the accept of integrated health care delivery system (IDSs) has gained considerable interest.
* Compare health care system between new Zealand and US base in the key point on orginal question
New Zealand health care system is comprehensive and largely publicly funded. It generally performs well ,but there are significant inequities in access and outcomes.
As international comparison show ,New Zealand health care is funded largely through taxation, and we spend less than other countries when measured by cost per person.
The New Zealand health service is generally seen to perform well.the structure of the service allows the government a good deal of control over total health spending and enables funding to follow key priorities.
International comparison show that our health system demonstrates good value for money. It delivers higher than average life expectancy at birth.Although it has reduced the total number of Dailys lost slightly faster than the high income country average, there are other countries that do better than New Zealand, perhaps due in part to New Zealands lower per capita cumulative spending in recent years.
The United states health care system is most expensive in the world, but this report and prior editions consistently show the US.underperform relative to other countries on most dimensions of performance.
Quality in to four categories; effective care safe care ,coordinated care, and patient- centered care.compared with the other 10 countries, the US .fares best on provision and receipt of preventive and patient - centered care.while there has been some improvement in recent years, lower scores on safe and coordinated care pull the overall U.S
Acesss: Not suprisingly- given the absence of universal coverage people in the U.S .go without needed health Care because of cost more often than people do in the other countries.Americans were the most likely to say they had access problems related to cost.
Efficiency; on indicators of efficiency the U.S ranks last among the 11 countries with the UK and Sweden ranking first and second, respectively.
Health lives : the U.S ranks last overall with poor score on all three indicators of healthy lives - mortality amenable to medicare ,infant mortality, and healthy lifes expectancy at age 60. The U.S and U.K had much higher death rate in 2007 from conditions amenable to medical care than some of the other countries.e.g,rates 25% to 50% higher than Australia and Sweden.overall ,France,Sweden, and Switzerland rank highest on healthy lives.
